This position is dedicated to improving the quality of life for a defined population by collaborating closely with the healthcare team. The primary mission is to facilitate the coordination of appropriate, cost-effective services that align with each member's plan of care. By focusing on psychosocial assessments, resource access, and advocacy, the role aims to help members achieve their optimal level of independence.
You will serve as a vital link between patients, families, and the broader healthcare system. Your day-to-day involves guiding members through complex health-related legal processes, providing supportive counseling on end-of-life issues and advanced directives, and ensuring that care plans are implemented efficiently. Whether coordinating family meetings, assisting with guardianship paperwork, or advocating for community resources, you will work to decrease inappropriate medical utilization while enhancing patient outcomes.
Candidates for this role are eligible for a $7,500 sign-on bonus. Please note that this position requires credentials from multiple states. Credentials from the primary work state must be secured at the time of hire, with additional credentials from secondary work states required within six months of employment.
