About the Role
This position is dedicated to improving the quality of life for a defined population by collaborating closely with the healthcare team. The primary mission is to facilitate the coordination of appropriate, cost-effective services that align with each member's plan of care. By focusing on psychosocial assessments, resource access, and advocacy, the role aims to help members achieve their optimal level of independence.
You will serve as a vital link between patients, families, and the broader healthcare system. Your day-to-day involves guiding members through complex health-related legal processes, providing supportive counseling on end-of-life issues and advanced directives, and ensuring that care plans are implemented efficiently. Whether coordinating family meetings, assisting with guardianship paperwork, or advocating for community resources, you will work to decrease inappropriate medical utilization while enhancing patient outcomes.
Hiring Process & Application
Candidates for this role are eligible for a $7,500 sign-on bonus. Please note that this position requires credentials from multiple states. Credentials from the primary work state must be secured at the time of hire, with additional credentials from secondary work states required within six months of employment.

Key Responsibilities
- check_circleEnsure consistent documentation of case management activities in compliance with organizational standards
- check_circleAnalyze patient and program outcomes to identify improvements in quality and cost effectiveness
- check_circleComplete guardianship paperwork and provide technology assistance for virtual court proceedings
- check_circleCoordinate and advocate for appropriate community resources to meet patient needs in a cost-effective manner
- check_circleProvide supportive counseling and education to members, families, caregivers, and health care professionals
- check_circleManage and coordinate assigned caseloads consistent with established clinical criteria
- check_circleConduct comprehensive psychosocial assessments to evaluate patient goals, health status, and functional limitations
- check_circleFacilitate applications for local, state, and federally funded programs such as Medicaid and Medicare
- check_circleSchedule and coordinate family meetings as needed to support patient care planning
- check_circleCollaborate with the health care team to develop and monitor individualized plans of care
Requirements
- verifiedMasters degree in social work (MSW) required
- verifiedMinimum one (1) years of clinical social work experience in a health care setting required
- verifiedLicensed Independent Social Worker (District of Columbia) within 6 months of hire OR Licensed Graduate Social Worker (District of Columbia) within 6 months of hire
- verifiedLicensed Master Social Worker (Maryland) within 6 months of hire
- verifiedLicensed Master's Social Worker (Virginia) within 6 months of hire
Nice to Have
Experience with computer software programs in a Windows environment. Knowledge of community systems and resources in the defined service area. Knowledge of regulatory issues for the Mid-Atlantic area.
