This position serves as a vital link within the healthcare team, dedicated to improving the quality of life for a defined patient population. The core mission is to facilitate the coordination of appropriate, cost-effective services that align with each member's plan of care. By focusing on psychosocial assessments, resource advocacy, and care planning, the role aims to help patients achieve their optimal level of independence while ensuring the efficient use of medical resources.
In this capacity, you will work closely with nurse case managers and other healthcare professionals to develop and monitor comprehensive care plans. Your day-to-day involves guiding members through complex health-related legal processes, connecting them with community resources, and providing supportive counseling to patients, families, and caregivers. The role also encompasses critical administrative duties, such as scheduling family meetings, completing guardianship paperwork, and assisting patients with technology for virtual court proceedings. For inpatient settings, the role includes specific compliance tasks regarding Uniform Assessment Instruments (UAIs) for long-term care.
This position is eligible for a $4,500 sign-on bonus.
Candidates must hold credentials from the primary work state at the time of hire. Additional credentials for secondary work states are required within six months of employment.
