About the Role

As a VNS Health Home Care LPN, you will provide one-on-one, individualized care under the direction of Registered Nurses. Your mission is to meet patients where they are, delivering exceptional clinical outcomes to our neighbors most in need. This role is designed for those interested in advancing their clinical careers; VNS Health actively supports professional growth, whether through expanding clinical knowledge or pursuing additional education to become a Registered Nurse.

In this position, you will practice in the community, traveling to patients' homes or care facilities. You will utilize agility and empathy to deliver continuity of care, coordinating supplies and providing education to patients and their families, including support for end-of-life treatment decisions. You will also leverage industry-leading technology, such as tablet computers and electronic medical record systems, to maintain accurate records and make informed decisions. The role offers a unique opportunity to contribute to clinical research and organizational projects while managing a weekly caseload with effective time management.
