VNS Health has been committed to meeting the needs of New Yorkers for over 130 years. As one of the largest nonprofit home- and community-based health care organizations in the country, more than 11,500 team members work together to make a difference in the lives of over 99,000 patients and members on any given day.
Our mission is to redefine the standard of patient-centered care, keeping New Yorkers out of crowded hospital systems so they can heal and age where they are most comfortable—in their homes and communities. We are building an innovative Future of Care, and we invite you to be part of our 130-year history.
As a Home Care LPN, you will provide one-on-one, individualized care under the direction of Registered Nurses. Your role is to meet patients where they are, delivering exceptional clinical outcomes to our neighbors most in need. You will practice in the community, traveling to patients' homes or care facilities to manage a weekly caseload with agility and empathy.
This position is designed for those interested in advancing their clinical careers. VNS Health supports and encourages professional growth, offering opportunities to expand clinical knowledge or pursue additional education to become a Registered Nurse. You will have the chance to contribute to clinical research and organizational projects while utilizing industry-leading technology to make informed decisions and deliver quality care.
We offer comprehensive training, including three weeks of paid clinical orientation, preceptorship, and ongoing skills development. Whether you are a new graduate or an experienced professional, we provide the support you need to succeed. To apply, please submit your credentials demonstrating your New York State LPN license and registration. Bilingual skills may be required depending on operational needs.
We are dedicated to the well-being of our team members. Beyond clinical excellence, we offer a supportive environment with internal mobility, generous tuition reimbursement, and CEU credits. Our culture is built on the belief that every team member plays a vital role in the health of our community.
