As a VNS Health Home Care LPN, you will provide one-on-one, individualized care under the direction of Registered Nurses. Your mission is to meet patients where they are, delivering exceptional clinical outcomes to those most in need. This role is designed for those interested in advancing their clinical careers; VNS Health actively supports professional growth, whether through expanding clinical knowledge or pursuing additional education to become a Registered Nurse.
You will practice in the community, traveling to patients' homes or care facilities. Your day involves monitoring patient progress, delivering personalized care management by adjusting plans as needs evolve, and coordinating with an interdisciplinary team to ensure effective care delivery. Using technology like tablets and electronic medical records, you will maintain accurate records while providing empowering education to patients and families, including support for end-of-life treatment decisions.
