As a Home Care LPN, you will provide one-on-one, individualized care with the direction of our Registered Nurses. You will meet patients where they are, delivering exceptional clinical outcomes to our neighbors most in need. This role is designed for those interested in advancing their clinical career; VNS Health supports and encourages professional growth, whether through expanding clinical knowledge or pursuing additional education to become a Registered Nurse.
In this position, you will practice in the community, traveling to patients' homes or care facilities. You will use agility and empathy to deliver continuity of care, coordinating supplies and providing education to patients and their families, including support in navigating end-of-life treatment. You will leverage industry-leading technology, such as tablet computers and electronic medical record systems, to maintain accurate records and make informed decisions. Your work will be supported by comprehensive training, including three weeks of paid clinical orientation and preceptorship, alongside ongoing skills development and opportunities to contribute to clinical research.
