As a Home Care LPN, you will provide one-on-one, individualized care under the direction of Registered Nurses. Your mission is to meet patients where they are, delivering exceptional clinical outcomes to those most in need. This role is designed for those interested in advancing their clinical careers; VNS Health actively supports professional growth, whether through expanding clinical knowledge or pursuing additional education to become a Registered Nurse.
In this position, you will practice in the community, traveling to patients' homes or care facilities. You will manage your weekly caseload with agility, using technology like tablets and phones to maintain accurate electronic medical records. Your day involves monitoring patient progress, delivering personalized care management by adjusting plans as needs evolve, and coordinating with an interdisciplinary team to ensure effective care delivery. You will also provide empowering education and support to patients and families, particularly regarding end-of-life treatment.
