Hebrew SeniorLife is dedicated to a philosophy that recognizes patient dignity and choice in all aspects of daily life. Our mission is to maximize the quality of life for our patients by fostering a culture of change and creating an inclusive environment where every encounter is positive and meaningful. We celebrate diversity and are committed to supporting a collaborative, multidisciplinary team that includes nurses, physicians, social workers, therapists, and dietitians.
The Licensed Practical Nurse (LPN) is an integral member of the clinical support team, working in close collaboration with Registered Nurses to deliver high-quality nursing care. In this role, you will utilize the nursing process of assessment, planning, implementation, and evaluation to meet the clinical needs of patients while ensuring safe and efficient care delivery.
You will serve as an active supporter of culture change initiatives, acting as a role model and mentor to patient care associates and other team members. Operating within a primary nursing model, you will maintain accountability for a set of assigned patients, collaborating with the RN to ensure care plans accurately reflect individualized needs. Your day-to-day responsibilities include documenting care in electronic medical records, identifying necessary resources, and participating in shared governance and performance improvement projects.
Depending on unit needs, you may also assume charge nurse responsibilities with RN oversight. In this capacity, you will provide clinical and supervisory oversight during a shift, delegate assignments, conduct shift reports, and assist with admission or discharge processes. You will also serve as a resource for the team, helping to develop new work processes and ensuring compliance with all policies, procedures, and regulatory standards, including HIPAA.
Candidates must be graduates of an approved school of nursing and hold a current Massachusetts license as an LPN in good standing. We are looking for individuals who are professional, proactive, and results-oriented, with excellent communication skills and the ability to adapt to changing priorities.
Hebrew SeniorLife is an equal opportunity employer. We celebrate diversity and are committed to creating an inclusive environment for all employees.
