The Licensed Practical Nurse (LPN) is an integral member of the clinical support team at Hebrew SeniorLife, working in collaboration with Registered Nurses and a multidisciplinary group of providers to deliver high-quality, patient-centered care. This role is designed to maximize the quality of life for patients by utilizing the nursing process of assessment, planning, implementation, and evaluation.
As an active supporter of culture change initiatives, the LPN serves as a role model and mentor to patient care associates and the broader team. The position requires a deep commitment to the organization's philosophy, recognizing patient dignity and choice in all aspects of daily life. The goal is to transform every patient encounter into a positive, meaningful experience while ensuring safe and efficient care delivery.
In this role, you will practice within a primary nursing model, maintaining accountability for a set of assigned patients under RN oversight. Your day involves collaborating with physicians, nurse practitioners, social workers, therapists, and dietitians to monitor outcomes and implement evidence-based interventions. You will document care in electronic medical records, initiate necessary regulatory reports, and participate in care planning activities that support patient autonomy.
You may also be assigned charge nurse responsibilities, which include giving shift reports, providing clinical oversight to the unit, delegating assignments, and auditing admission or discharge processes. Beyond direct patient care, you will actively participate in shared governance committees, performance improvement projects, and unit activities, serving as a resource for the team and a coach for new staff.
