About the Role
Interim HealthCare is seeking a Licensed Medical Social Worker to provide compassionate, patient-centered support to individuals receiving home health services. This role exists to help patients and families navigate the complex medical, emotional, and social challenges that often accompany home health care. You will serve as a vital link between clinical teams and the community, ensuring that care extends beyond the medical treatment plan to address the whole person.
In this position, you will conduct psychosocial assessments, develop individualized care plans, and coordinate essential community resources. Your day will involve close collaboration with nurses, therapists, and physicians to support safe transitions of care and promote optimal outcomes for every patient you serve.
