This position is dedicated to supporting a defined population by collaborating closely with the healthcare team. The primary mission is to facilitate the coordination of appropriate, cost-effective services that align with each member's plan of care. The role focuses on helping individuals achieve their optimal level of independence while enhancing their overall quality of life.
You will serve as a vital advocate, guiding members through complex health-related legal processes and connecting them with essential community resources. This includes conducting comprehensive psychosocial assessments to evaluate patient goals, social support systems, and functional limitations. By analyzing outcomes and identifying areas for improvement, you will help ensure the efficient and timely implementation of care plans that reduce inappropriate medical utilization.
The role involves providing supportive counseling and education to members, families, caregivers, and the broader healthcare team. Key responsibilities include navigating end-of-life issues, Advanced Directives, and guardianship paperwork. You will also play a critical role in scheduling family meetings and providing technology assistance to ensure patients and families can virtually attend court proceedings when necessary.
This is an inpatient-only position requiring a commitment to alternating weekends and holidays. The role demands a high level of adaptability to manage a caseload consistent with established criteria while maintaining rigorous documentation standards.
Candidates must possess credentials from the primary work state at the time of hire. Additional credentials from secondary work states (District of Columbia, Maryland, and Virginia) are required within six months of employment. The position specifically requires an Independent Clinical Social Worker License (DC), a Licensed Certified Social Worker (Maryland), and a Licensed Clinical Social Worker (Virginia) to be obtained within the first six months.
