This position exists to support patients and their families in improving their quality of life and enhancing their ability to function successfully within the community.
In this role, you will serve as a vital link between clinical care and community integration. Your day will involve conducting risk assessments to ensure patient safety, facilitating access to medical appointments and therapeutic activities, and collaborating with treatment teams to refine care plans. A significant portion of your time will be dedicated to documentation, ensuring all services meet medical necessity guidelines and are recorded with precision and timeliness.
