Key Responsibilities

  • check_circleCoordinate follow-up services for hospitalizations and facilitate smooth transitions of care
  • check_circleMonitor HEDIS gaps and verify client payer and program enrollment status monthly
  • check_circleAssist the Nurse Practitioner with clinic documentation, vitals, and med management education
  • check_circleEngage in outreach to support marketing initiatives and facilitate new client enrollment
  • check_circleMaintain medical record compliance and ensure timely documentation of all care coordination activities
  • check_circleEstablish collaborative relationships with care teams and community resources to improve resource linkage
  • check_circleConduct comprehensive assessments and develop person-centered care plans addressing unmet health needs
  • check_circlePlan, coordinate, and monitor individualized treatment plans to align with behavioral health goals
  • check_circleManage clinic roster and census data to ensure accuracy with Carelogic system
  • check_circleIdentify and provide crisis response while participating in post-crisis debriefing

Requirements

  • verifiedBachelor's Degree in any discipline (TN)
  • verifiedExperience working with children and families in case management (TN)
  • verifiedMH/SU license or NC Substance Abuse Board certification (NC)
  • verifiedRN with 4 years MH/SU/IDD experience (NC)
  • verifiedMaster's degree in human service with 1 year MH/SU/IDD experience (NC)
  • verifiedBachelor's degree in human service with 2 years MH/SU/IDD experience (NC)
  • verifiedBachelor's degree in non-human service with 4 years MH/SU/IDD experience (NC)
  • verified2 years experience with behavioral health conditions (NC)
  • verified2 years LTSS/HCBS care management experience for LTSS members (NC)
  • verifiedPass comprehensive background investigation
