As an Intake Specialist, you will be the initial point of contact for new service recipients, facilitating enrollment via telephone or face-to-face interactions. Your primary mission is to develop the initial integrated comprehensive assessment and Integrated Service Plan (ISP) within required timeframes. You will guide new members through the intake packet process, including consents, member handbooks, and privacy notices, while assisting them with Patient Portal enrollment. This role serves as a critical bridge, ensuring that all new referral information is accurately provided to the Integrated Case Manager and care team to coordinate ongoing healthcare, behavioral health, and wellness services.
