About the Role
The Inpatient Discharge Planner serves as a vital bridge between the clinical team and the patient's journey home. Based on Unit C3, you will partner closely with nursing staff, social workers, and providers to craft comprehensive discharge plans that ensure continuity of care. Your role is to keep the care team, patients, and families informed and aligned throughout the process.
Success in this position requires a deep understanding of discharge protocols and utilization criteria, ensuring that every patient's needs are met with the appropriate resources at the moment of discharge. You will also maintain awareness of the continuum of care that extends beyond the inpatient stay, facilitating a smooth transition for patients and their support systems.
