About the Role
The Individual Support Case Manager serves as a vital partner to MassAbility participants, dedicated to fostering their independence and well-being. This role focuses on developing, maintaining, and maximizing skills in self-care, physical and emotional growth, socialization, communication, and vocational areas. The ultimate goal is to support participants in establishing and maintaining a residence while living fully within the community.
Day in the Life
You will work closely with a multidisciplinary team to ensure comprehensive care for each participant. Your day involves conducting home visits, documenting activities, and providing resources that help individuals maintain safety and independence. A significant portion of your time will be spent training and supervising caregivers and Personal Care Assistants (PCAs) to ensure high-quality support. You will also manage necessary administrative duties, including accurate and timely documentation, while ensuring your vehicle is maintained for safe client transport.
