The Housing Specialist (HUD & ESSHI) plays a vital role in supporting individuals and families across Onondaga and Madison Counties who may be experiencing homelessness, serious mental illness, or co-occurring disorders. This position is designed to facilitate and maintain independent housing by providing direct support, teaching, and coordinating daily living activities that promote full and meaningful community integration.
In this collaborative role, you will work closely with Onondaga's CoC and Coordinated Entry to locate and house chronically homeless individuals and families. You will establish individualized service plans that target barriers to stable housing, working in partnership with clients and other service providers to implement goals. The role involves a dynamic day-to-day rhythm of outreach, assessment, and crisis intervention, ensuring continuity of services while helping clients achieve financial independence and maintain their permanent housing.
