The Permanent Supportive Housing Program serves Fulton County residents, including primary tenants and their families. Designed as a cost-effective solution, the program helps individuals with disabilities achieve more stable and productive lives. It is widely recognized as a key to success for those facing complex challenges, including homelessness, very low to no income, and serious persistent issues such as substance abuse, mental illness, HIV/AIDS, or other significant barriers to a successful life.
River Edge is seeking an energetic and enthusiastic Full-time Care Coordinator for our Fulton Permanent Supportive Housing initiative. This role is vital for fostering a client-centered culture and assisting individuals with substance abuse and mental health referrals, housing applications, and other housing needs. The Care Coordinator will work closely with Case Managers to support a caseload of up to 20 individuals, conducting in-home visits and maintaining confidential behavioral health assessments. The position requires a strong commitment to helping clients develop solution-focused treatment plans and navigate crisis situations.
All applicants must pass a satisfactory background clearance and a pre-employment drug test. Candidates are required to provide two professional reference statements from former employers (family members are not accepted). The interview process will evaluate expertise in mental health and substance abuse services, as well as communication skills.
