Founded in 1967, The HealthCare Connection (THCC) was Ohio's first Federally Qualified Health Center (FQHC). Our mission is to provide quality and accessible primary healthcare services through community-responsive approaches that address financial, geographic, and other barriers to care for residents of northern Hamilton County and surrounding areas. THCC is proudly recognized as a Level 3 Patient Centered Medical Home (PCMH), the highest level of recognition attainable for quality care.
We operate two primary care locations and six school-based health centers, providing value-based care for over 20,000 patients. Our integrated services span Primary Care, Infectious Disease, Substance Use, Integrated Behavioral Health, Dental Services, Women's Health, and Pharmacy.
THCC is seeking a School-Based Behavioral Health Provider for a unique opportunity to serve children, adolescents, and families within our School-Based Health Centers. This is a rewarding role ideal for a licensed clinician eager to make a lasting impact on the lives of underserved communities through integrated care.
In this position, you will work within our Norwood and North College Hill school locations. You will partner closely with primary care providers to deliver brief, solution-focused assessments and interventions aimed at functional restoration. Your day will involve managing the psychosocial aspects of chronic and acute diseases, providing crisis intervention, and conducting substance abuse screenings. You will also serve as an educator, consulting with the primary care team on mental health and behavioral medicine while supporting patients and families with self-management and harm reduction strategies.
