Southern Health Partners Inc is seeking an LPN Medical Team Administrator to deliver compassionate clinical care and coordinate daily medical operations within correctional facilities. This role exists to ensure high-quality healthcare delivery in a secure, structured environment where patient safety and regulatory compliance are paramount.
In this position, you will serve as a vital link between patients, providers, and custody staff. Your day will involve providing direct patient care, including assessments, treatments, and patient education, while administering medications and immunizations in strict adherence to facility and state guidelines. You will assist providers with sick call, chronic care clinics, and urgent patient needs, ensuring that care is prioritized based on acuity.
Beyond direct clinical duties, you will play a key role in maintaining accurate electronic and paper medical records and coordinating appointments and referrals. By collaborating with an interdisciplinary team, you will help promote a safe, respectful environment and contribute to quality improvement and infection control initiatives. Effective communication with custody staff is essential to support a secure clinical atmosphere, fostering a team-oriented culture that values compassion for both patients and staff.
