Founded in 1990, Housing Works is a pioneer in AIDS advocacy and the social entrepreneurship movement. We operate 10 high-end thrift shops in Manhattan and Brooklyn, a beloved Bookstore Cafe in Soho, and provide lifesaving services including New York State's first harm reduction-based drug treatment program, health home care management, and syringe exchange programs. The Housing Case Manager (HCM) serves as a vital partner in the STARS 6 Young Adult family program, a scattered-site supportive housing initiative designed for young families. This role exists to ensure housing stability by working directly with clients in their homes, the community, and the office. You will help clients transition from homelessness to stable housing while promoting independent living skills, financial management, and overall health and wellness.
Your day involves building strength-based relationships with clients to develop individualized goals, assisting with the move-in process, and co-facilitating parenting and group activities. You will act as a bridge between clients and a vast array of internal and external providers, coordinating care across behavioral health, medical, legal, and job-training sectors. The role requires traveling via mass transit to accompany clients to appointments and providing crisis intervention and mediation as needed.
