As a Hospice RN / RN Case Manager, you will be the primary point of contact for patients and families, guiding them through every step of their hospice journey with dignity, comfort, and love.
You will deliver skilled, hands-on nursing care in diverse settings, including private homes, assisted living facilities, and wherever our patients call home. Your role involves collaborating with an interdisciplinary team—including physicians, social workers, and chaplains—to build and refine personalized care plans. Beyond clinical duties, you will provide emotional, physical, and spiritual support to both patients and their loved ones, offering education on medication administration, symptom management, and end-of-life expectations. You will also teach caregivers how to care for their loved ones safely and confidently, ensuring timely interventions and maintaining the highest standards of clinical documentation.
