As a Hospice Registered Nurse, you will join a company where every day is an opportunity to deliver compassionate care to patients facing life-limiting illness. You will serve as the primary point of contact for patients and families, guiding them through every step of their hospice journey with dignity, comfort, and love.
Your day will involve delivering and documenting skilled, hands-on nursing care based on each patient’s individualized Plan of Care, whether in their home, an assisted living facility, or wherever they call home. You will perform initial and ongoing assessments to monitor patient conditions and adjust care as needed. Working closely with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, and chaplains—you will build and refine personalized care plans that address the emotional, physical, and spiritual needs of both the patient and their loved ones.
Beyond clinical duties, you will provide education and comfort to families, teaching caregivers how to care for their loved ones safely and confidently. You will also help patients and families navigate complex topics such as medication administration, hospice philosophy, symptom management, and end-of-life processes. Your role is vital in ensuring care is consistent, compassionate, and efficient while maintaining accurate clinical documentation and compliance standards.
