About the Role
Care 360 Hospice is seeking dedicated LPNs and RNs to join our team in Hilliard, Ohio, and the surrounding area. In this after-hours position, you will provide skilled nursing care to patients at the end of life, conducting visits in both home and facility settings. This role is vital for ensuring continuity of care when patients and families need support most, offering on-call services and responding to urgent concerns regarding symptoms, condition changes, or emergencies.
As a Hospice Nurse, you will work a full-time schedule on Thursday, Friday, and Saturday nights (8 PM–8 AM). You will collaborate closely with the RN Case Manager (RNCM) to execute and maintain the patient plan of care, assist with specialized procedures, and reinforce education for patients and families regarding end-of-life care. The position requires active participation in professional growth through in-service programs, workshops, and conferences, ensuring you stay current with best practices in hospice and palliative care.

Requirements
