Are you passionate about End of Life Care and enjoy providing support to patients and their families? As a Hospice Registered Nurse Case Manager with BJC Home Care, you will be part of a special kind of caring for patients with a life-limiting illness. Services are provided in the comfort of the patient's home or skilled nursing facility.
Our Hospice staff are sensitive to the physical, psychosocial, emotional, and spiritual needs of terminally ill adult and pediatric patients and their families. You will work as part of a multi-disciplinary team of healthcare professionals and volunteers, specially trained in symptom management, pain control, counseling, and bereavement services. Our programs also provide palliative care by helping patients manage their pain and symptoms while living their lives with daily peace and dignity, utilizing alternative therapies such as music, art, and massage therapy.
In this role, you will evaluate clients and furnish services requiring substantial and specialized skill. When assigned as a case manager, you are responsible for coordinating all aspects of care related to that patient, developing individual plans of care with defined goals, and ensuring evidence-based practice and quality outcomes.

This position operates Monday through Friday from 8:00 am to 4:30 pm, with a weekend and on-call rotation. You must be willing to provide coverage in all St. Louis regions during on-call shifts. The role includes a cell phone and laptop, mileage reimbursement at the IRS rate, and relocation availability for eligible candidates. A sign-on bonus of up to $15,000 is available.
