CenterWell Home Health is a leading healthcare services business focused on creating integrated and differentiated experiences that put patients at the center of everything we do. As part of Humana Inc., we specialize in personalized, comprehensive home care for patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization. Our care teams—including nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers—work together to help patients rehabilitate, recover, and regain their independence so they can live healthier, happier lives.
As a Home Health Speech Language Pathologist, you will become a vital part of our caring community. Reporting to the Clinical Manager, your mission is to help patients regain strength, mobility, and independence from the comfort of their homes. By delivering personalized care focused on rehabilitation and functional improvement, you will empower individuals to overcome physical limitations, perform everyday activities with confidence, and enjoy a better quality of life.
In this role, you will evaluate, direct, and provide speech/language pathology services to patients in home or facility settings. You will utilize your professional judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing, aiming to prevent, identify, and minimize their effects. Your day-to-day will involve administering diagnostic tests, interpreting therapeutic treatments, and providing instruction on alternative communication systems. You will also counsel patients, families, and healthcare staff, while maintaining the highest standards of professional conduct regarding patient confidentiality.
This role is patient-facing and part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to undergo screening. Additionally, as this position requires travel to patient locations, it is part of Humana's driver safety program. Candidates must possess a valid state driver's license and maintain personal vehicle liability insurance in accordance with state minimum limits or specific coverage thresholds ($25,000 bodily injury per person/event and $10,000 property damage, or whichever is higher).
Humana is an Equal Opportunity Employer. It is our policy not to discriminate against any employee or applicant for employment because of race, color, religion, sex, sexual orientation, gender identity, national origin, age, marital status, genetic information, disability, or protected veteran status. We take affirmative action to employ and advance individuals with disabilities or protected veteran status, basing all employment decisions only on valid job requirements.
