As a Home Health Speech Language Pathologist, you will evaluate, direct, and provide speech/language pathology services to patients in the home or facility. You will use your professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing to prevent, identify, evaluate, and minimize the effects of such disorders and conditions.
Your day will involve administering and interpreting diagnostic tests, providing instruction and training to patients in the use of alternative communication systems, and offering counsel to patients, families, and healthcare staff. You will participate in the development and periodic review of the Plan of Treatment and Plan of Care, while observing, recording, and reporting changes in the patient's condition to your supervisor and/or the physician. Additionally, you will participate in care coordination activities and discharge planning, ensuring the highest standards of professional conduct regarding confidential patient information.
