As a Home Health Speech Language Pathologist, you will evaluate, direct, and provide speech/language pathology services to patients in the home or facility. You will use your professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing to prevent, identify, evaluate, and minimize the effects of such disorders and conditions.
Your day will involve administering and interpreting diagnostic tests, therapeutic treatments, and audio logic screenings. You will observe, record, and report changes in a patient's condition and response to treatment to your supervisor and/or physician. Additionally, you will provide instruction and training to patients in the use of alternative communication systems when appropriate, and offer counsel and instruction to patients, families, and healthcare staff.
You will participate in the development and periodic review of the Plan of Treatment and Plan of Care, as well as care coordination activities and discharge planning. Maintaining the highest standards of professional conduct regarding confidential information is essential; you will share information only when the recipient's right to access is clearly established and the sharing is in the best interests of the patient.
This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
As this position requires travel to patient locations, it is part of Humana's driver safety program. You must have a valid state driver's license and maintain personal vehicle liability insurance in accordance with your residing state's minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher.
