As a Registered Nurse on our Home Health team, you will serve as the primary clinical partner for patients and families in the Cedar Rapids area and surrounding communities. Your mission is to deliver individualized, one-on-one nursing care that promotes recovery, safety, and independence. You will conduct comprehensive in-home assessments, translate findings into thoughtful care plans, and provide skilled interventions tailored to each patient's unique goals.
This role is designed for nurses who value continuity and want to truly know their patients. You will work Monday through Friday during daytime hours, with participation in a shared on-call rotation. By monitoring patient progress and communicating effectively with physicians and the interdisciplinary team, you will help prevent avoidable hospitalizations and ensure high-quality outcomes throughout the patient's home health journey.
