As a CenterWell Home Health nurse, you will join a dedicated team of physicians and clinicians to provide personalized, one-on-one care. Your mission is to help patients regain independence in the comfort of their own homes, managing care plans that support recovery and enable them to return to the life they love.

Key Responsibilities
- check_circleProvide skilled nursing care to patients onsite in their homes

- check_circleConduct patient assessments and monitor their conditions
- check_circleDevelop and implement individualized care plans with physicians
- check_circleEducate patients and families on disease management and treatment
- check_circleMaintain accurate records of patient care and coordinate with professionals
- check_circleOversee the work of Home Health Aides and monitor documentation
Requirements
- verifiedMed surg, ICU, ER, or acute experience
- verifiedCurrent and unrestricted Registered Nurse licensure
- verifiedCurrent CPR certification
- verifiedExperience collaborating with a team of healthcare professionals
- verifiedValid driver's license, auto insurance and reliable transportation
Nice to Have
One year nursing experience. Home Health experience.
