As a CenterWell Home Health nurse, you will join a dedicated team of physicians and clinicians to provide personalized, one-on-one care. In this role, you will serve as the primary clinical partner for your patients, working onsite in their homes. You will manage care plans, administer medications, perform wound care, and conduct assessments while educating patients and their families on disease management and treatment options. A key part of your day involves collaborating with physicians and other healthcare professionals to monitor patient conditions, report changes, and oversee the work of Home Health Aides to ensure documentation is accurate and complete.
This role is patient-facing and is part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to undergo TB screening. Additionally, as this position requires travel to patient homes, it is part of Humana's driver safety program. Candidates must possess a valid state driver's license and maintain personal vehicle liability insurance in accordance with state minimums or specific coverage limits.
