About the Role

As a Home Health Physical Therapist, you will work onsite in patients' homes to attain the highest level of physical function. You will assess and document patients' physical strengths to support physician evaluations and guide treatment goals. Your role involves developing and implementing rehabilitation programs consistent with the physician's Plan of Treatment and the goals of the patient team. Additionally, you will provide clear written care instructions to Physical Therapist Assistants (PTAs) and Home Health Aides, overseeing their performance and documentation. You will serve as a vital link by reporting patient progress to physicians, the Clinical Manager, the care team, patients, and families, ensuring all required evaluations, plans, progress notes, and discharge summaries are submitted promptly.

Important Role Information

This role is patient-facing and is part of Humana's Tuberculosis (TB) screening program. If selected, you will be required to be screened for TB.

This position is part of Humana's driver safety program. It requires a valid state driver's license and the expectation that you maintain personal vehicle liability insurance. You must carry vehicle insurance in accordance with your residing state's minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher.
