As a Home Health Physical Therapist, you will work onsite in patients' homes to attain the highest level of physical function. You will assess and document patients' physical strengths to support physician evaluations and guide treatment goals. Your role involves developing and implementing rehabilitation programs consistent with the physician's Plan of Treatment and the goals of the patient team. Additionally, you will provide clear written care instructions to Physical Therapist Assistants (PTAs) and Home Health Aides, overseeing their performance and documentation. You will also report patient progress to the physician, Clinical Manager, care team, patient, and family, ensuring all required evaluations, plans, progress notes, and discharge summaries are submitted promptly and thoroughly.

Tuberculosis Screening: This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Driving Requirements: This role is part of Humana's driver safety program and requires an individual to have a valid state driver's license. You are expected to maintain personal vehicle liability insurance in accordance with your residing state's minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher.
