As a Home Health Physical Therapist, you will join a team focused on empowering individuals to overcome limitations and enjoy a better quality of life. Reporting to the Clinical Manager, your mission is to help patients regain strength, mobility, and independence from the comfort of their own homes.
You will deliver personalized care that focuses on functional improvement, planning and administering prescribed physical therapy treatments onsite. By assessing patient strengths and developing rehabilitation programs consistent with physician plans, you will guide treatment goals and collaborate with a team of healthcare professionals to ensure the highest level of physical function. Your work involves providing clear care instructions to PTAs and Home Health Aides, overseeing their performance, and maintaining thorough documentation of patient observations and progress.
This role is part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to be screened for TB. Additionally, as this position involves driving to patient homes, it is part of Humana's driver safety program. Candidates must possess a valid state driver's license and maintain personal vehicle liability insurance in accordance with state minimum limits or specific coverage requirements.
