About the Role

Reporting to the Clinical Manager, the Medical Social Worker is a vital member of the interdisciplinary care team dedicated to addressing the emotional and social needs of home health patients. This role goes beyond clinical treatment to evaluate and develop personalized plans of care that support a patient's overall well-being. You will function as a resource for the team, identifying and resolving social barriers that may impede recovery, while providing direction and supervision to Social Worker Assistants.

In this position, you will assess the patient's social and emotional state in relation to their illness, evaluate their financial resources and community support systems, and coordinate necessary actions to secure resources such as housing, legal aid, or financial assistance. Your day-to-day involves communicating closely with patients, families, and associates to ensure a shared understanding of the treatment process and coping strategies. You will also maintain rigorous documentation and participate in care coordination activities to ensure compliance with company policies, healthcare standards, and governmental regulations.

Hiring Process & Application

This role is part of Humana's comprehensive safety and screening programs. Selected candidates will be required to undergo Tuberculosis (TB) screening as this is a patient-facing position. Additionally, candidates must possess a valid state driver's license and maintain personal vehicle liability insurance in accordance with state minimums or specific coverage limits ($25,000/$25,000/$10,000) to fulfill the driving requirements of the role.
