About the Role
Reporting to the Clinical Manager, the Medical Social Worker is a vital member of the interdisciplinary care team. This role exists to evaluate and develop personalized plans of care that address the emotional and social needs of home health patients. You will function as a resource to other health team members, identifying and resolving patient needs while ensuring compliance with company policies, healthcare standards, and governmental regulations.
In this position, you will assess the patient's social and emotional state in relation to their illness, financial resources, and available community resources. A key part of your day involves coordinating with patients, families, and associates to obtain resources that resolve issues impeding recovery. You will also provide direction and supervision to Social Worker Assistants, evaluating their performance to ensure all medical social services meet professional and compliance standards. Your work requires maintaining detailed documentation, participating in care coordination activities, and communicating effectively to support the patient's journey toward a healthier life.
Hiring Process
This role is part of Humana's Tuberculosis (TB) screening program and driver safety program. Selected candidates will be required to undergo TB screening and must possess a valid state driver's license with personal vehicle liability insurance meeting specific minimum limits.
