The Licensed Practical Nurse (LPN) is an essential part of the team responsible for providing and documenting skilled nursing care in accordance with the developed care plan and physicians' orders for each individual patient. This role requires adhering to confidentiality standards and professional boundaries at all times while delivering quality patient care in the comfort of their home.
You will work as part of a clinical family of clinicians who collaborate to meet the needs of each patient. The position offers the flexibility to build strong rapport with patients, which is key to producing the best clinical outcomes. This is a full-time position requiring Monday through Friday visits for the Chatham, Pooler, and Savannah communities.
