As a CenterWell Home Health nurse, you will become a vital part of our caring community. Reporting to the Clinical Manager, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their own homes. Working alongside a dedicated team of physicians and clinicians, you will manage care plans that support recovery and help patients return to the life they love.
In this role, you will implement individualized care plans in collaboration with the patient, family, and healthcare team. You will educate patients and their families on disease management, treatment options, and self-care techniques while maintaining accurate records of patient care. Your work involves maintaining prompt contact with patients, physicians, and clinical managers regarding status changes and participating in care coordination activities and discharge planning.
This role is patient-facing and is part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to be screened for TB. Additionally, as this position involves driving to patient homes, it is part of Humana's driver safety program. Candidates must possess a valid state driver's license and maintain personal vehicle liability insurance in accordance with their residing state's minimum required limits (or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher).
