About the Role
As a Home Health LPN, you will provide skilled nursing care to clients in accordance with the physician's plan of care and the supervising registered nurse's assignments. This role is designed for nurses who want to practice at the top of their license with greater flexibility, autonomy, and meaningful 1:1 patient relationships. Whether you are experienced in home health or transitioning from hospital, acute care, or skilled nursing settings, you will find a supportive environment that values your expertise.
You can expect consistent and reliable workloads with predictable schedules, eliminating the scramble for hours. The role features no on-call requirements, ensuring true work-life balance. You will utilize the Epic EHR system for efficient documentation, allowing you to spend less time charting and more time caring. Leadership is nurse-led, with accessible managers who understand the field and have your back.
