As a CenterWell Home Health nurse, you will become a vital part of our caring community. Reporting to the Clinical Manager, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working alongside a dedicated team of physicians and clinicians, you will manage care plans that support recovery and help patients return to the life they love.
This role is designed for those who want to use their skills to make a tangible impact. You will be the face of our mission, delivering skilled nursing care directly to patients in their homes under the direction of an RN. Your day will involve collaborating with patients and their families to implement individualized care plans, educating them on disease management and self-care, and maintaining accurate records of patient progress. You will also serve as a key communicator, promptly coordinating with physicians and the clinical team to address status changes and manage discharge planning.
This role is patient-facing and is part of Humana's Tuberculosis (TB) screening program; selected candidates will be required to undergo TB screening. Additionally, as this position involves driving to patient homes, it is part of Humana's driver safety program. Candidates must possess a valid state driver's license and maintain personal vehicle liability insurance in accordance with state minimums or specific coverage limits.
