As a CenterWell Home Health nurse, you will become a vital part of our caring community. Reporting to the Clinical Manager, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their own homes. Working alongside a dedicated team of physicians and clinicians, you will manage care plans designed to support recovery and help patients return to the life they love.
This role is designed for those who want to use their skills to make a tangible impact. You will be the face of our mission, delivering skilled nursing care onsite in patients' homes under the direction of an RN. Your day will involve implementing individualized care plans in collaboration with patients and their families, educating them on disease management and self-care techniques, and maintaining accurate records of patient care. You will also serve as a key communicator, promptly coordinating with patients, physicians, and the clinical team regarding status changes and discharge planning.
To ensure the safety and well-being of our patients and community, selected candidates will be required to participate in specific screening programs. This role is patient-facing and part of Humana's Tuberculosis (TB) screening program; if selected, you will be required to be screened for TB. Additionally, as this position requires travel to patient homes, it is part of Humana's driver safety program. Successful candidates must maintain a valid state driver's license and personal vehicle liability insurance in accordance with state minimums or specific coverage limits.
