About the Role
As a Home Health Clinical Manager, you will serve as the operational and clinical leader for our in-home care team. This role exists to ensure that every patient receives safe, high-quality care through effective oversight of clinical operations, care planning, and staff supervision. You will be the bridge between field clinicians, physicians, and families, coordinating admissions, recertifications, and discharges to optimize patient outcomes.
In this position, you will drive performance improvement initiatives and lead quality assurance efforts to maintain strict regulatory compliance. Your day will involve mentoring field clinicians, managing caseload distribution, and ensuring accurate documentation within EMR systems. Beyond the operational duties, you will champion a culture of compassion, continuously supporting your team's professional development and addressing the unique needs of our patients and their families.
