About the Role

We are seeking a Home Care Registered Nurse (Flex/PRN) to join our team within our gated community. This role is unique because it eliminates the need for driving; all clients are residents of our community. The RN in Home Care provides nursing care according to established policies and procedures for our Medicare-certified home care agency.

In this position, you will partner and collaborate during initial evaluations to assess residents' needs, developing and implementing service plans in conjunction with the resident, family, physician, and/or authorized practitioner. You will be the voice for the resident by communicating any changes in their condition and ensuring that their individualized plan is implemented and followed by the Home Health and Home Support agency team. Additionally, you will deliver nursing care as outlined in the plan of care according to acceptable nursing standards, participate in care conferences and resident-related meetings, and assist in the orientation and training of other agency nurses and aide staff development activities.
