The HIM / Scheduler serves as the operational backbone of St. Croix Hospice, ensuring the seamless coordination of care for patients and their families. This role is vital to maintaining the continuity of care, acting as the primary liaison between field staff, patients, physicians, and clinical supervisors. By managing complex scheduling logistics and maintaining rigorous data integrity, the Scheduler directly impacts the quality of life for those we serve.
In this position, you will work closely with our leadership team to optimize caregiver routes and ensure that every patient receives timely, compassionate care. The role offers a unique opportunity to work one-on-one with patients and their families, contributing to a mission-driven environment where many leaders have grown from within the company.
Your day will revolve around the electronic software system, where you will schedule all disciplines and manage patient records for billing purposes. You will utilize mapping tools to ensure field staff are positioned efficiently near their patients, while simultaneously managing schedule changes due to staff absences or missed visits.
Beyond scheduling, you will maintain strict HIPAA compliance through ongoing risk assessments and training initiatives. You will also manage supply inventory, process travel vouchers, and handle critical documentation such as OASIS collections and 485–486 forms. The role requires a high degree of organization, from date-stamping clinical notes daily to preparing agendas for interdisciplinary group (IDG) conferences.
