Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. We are a group of passionate people dedicated to delivering the highest quality services to our patients, families, team members, and the community. Our team is growing, and we are looking for a dedicated Physical Therapist to join our mission.
About the Role
As a Physical Therapist, you will facilitate the rehabilitation of patients with emotional, physical, or mental disabilities within the comfort of their homes. You will implement care plans, coordinate with the broader home healthcare team, and educate patients and family members about their health conditions. This role requires effective communication with patients, caregivers, and healthcare providers, as well as the preparation of timely and accurate clinical notes.
We currently have openings for both Full-Time and PRN shifts. Full-Time positions are available in the Northeast service area, while PRN positions are open in the Southwest.
