This full-time opportunity involves rounding in skilled nursing facilities across the Dallas area, including Irving and Arlington. You will provide high-quality care to a diverse patient population, collaborating with an experienced multidisciplinary team to build meaningful relationships with patients and their families.
This role offers the unique opportunity to deliver exceptional care during life's pivotal moments. You will work with autonomy and excellent support, utilizing innovative population health data reports to guide optimal and timely care. As part of TeamACO, a dedicated long-term Accountable Care Organization participating in the Medicare Shared Savings Program, clinicians may receive a portion of shared savings if certain criteria are met.
