We are seeking a quality-driven Nurse Practitioner or Physician Assistant to join our post-acute care team in Brooklyn, New York. This full-time opportunity involves rounding in skilled nursing facilities 4-5 days a week. You will provide high-quality care to a diverse patient population, collaborating with an experienced team to build meaningful relationships with patients and their families. This role offers the chance to deliver exceptional care during life's pivotal moments, supported by autonomy and excellent training.

Additionally, the Medical Director Leadership Academy (MDLA) positions you for success in quality improvement initiatives and collaboration with facility leadership.
