About the Role
We are seeking a quality-driven Nurse Practitioner or Physician Assistant to join our post-acute care team in Brooklyn, New York. This full-time opportunity involves rounding in skilled nursing facilities 4-5 days a week. You will provide high-quality care to a diverse patient population, collaborating with an experienced team to build meaningful relationships with patients and their families. This role offers the chance to deliver exceptional care during life's pivotal moments, supported by autonomy and excellent training.
Our innovative population health data reports will guide you toward optimal and timely care for our patient population. Additionally, the Medical Director Leadership Academy (MDLA) positions you for success in quality improvement initiatives and collaboration with facility leadership. This best-in-class educational program is not provided by any other organizations practicing post-acute medicine.

You will be paired with a dedicated clinical field mentor and receive ongoing support from industry-leading clinical experts in post-acute medicine.
