About the Role

We are seeking a compassionate and skilled Bilingual Advanced Practice Provider (NP/PA) to deliver high-quality in-home care to patients living with multiple chronic conditions, including diabetes, hypertension, chronic kidney disease, heart failure, COPD, and depression. This role works within an integrated multidisciplinary care team (Clinical, Behavioral Health, Pharmacy, Administrative), establishing long-term patient relationships and providing continuity of care to complex patients in their homes and via telehealth.

As an Advanced Practice Provider at Monogram Health, you will be part of a value-based, patient-centered care model, providing quality-driven managed care with a population health focus. You will have the opportunity to grow your clinical skillset through a multi-specialty training program designed for complex patient care. Your day will involve conducting 4–6 completed visits, each lasting 30–90 minutes, with a schedule of Monday–Friday, 8am–5pm. There is no on-call, overnight, or weekend work, including paid company holidays.
