You will join the Inclusa team in Dunn County, WI, to support members in the Wisconsin Family Care program. Reporting to the Supervisor of Care Coaching, you will serve as a vital link between members and the care system. Your primary mission is to assess member needs and eligibility using the State Department of Health Services (DHS) screening tool. This role involves conducting in-home, face-to-face interviews to evaluate functional abilities, health status, and required supports. You will determine the appropriate level of care, ensuring assessment data aligns with internal records and is submitted promptly according to state guidelines. This position is field-based, requiring you to travel up to 50% throughout Dunn County to reach frail elders and adults with intellectual, developmental, or physical disabilities.
