Since 1996, Community Healthcare of Texas has been dedicated to providing compassionate hospice and palliative care services. Our mission is to support individuals living with serious illnesses while caring for their loved ones who provide daily support.
We are currently recruiting for a Field LVN to join our team. In this role, you will assist the RN Case Manager in developing and executing the plan of care for patients. Your day-to-day responsibilities involve coordinating with team members and communicating with the interdisciplinary group (IDG) to ensure care aligns with organizational policies and regulations.
You will be responsible for administering medications and treatments as prescribed, maintaining accurate clinical notes regarding patient conditions and care outcomes, and documenting significant interactions with patients and their support persons. A key part of your role includes assessing patient and family responses to care and participating in follow-up contacts with bereaved families. You will manage an assigned caseload equally, regardless of the severity or complexity of the diagnosis, and participate in an on-call rotation.
